Find a wide range of features at your fingertips. You can:
- See your health history, medicines, suggested screenings and tests, scanned documents, X-rays and other images, plus more.
- Download your records and share them with health care professionals at other organizations.
- Link medical records from providers outside Optum Care Washington to your MyChart record. This helps create a more complete record of your health care history, all in one place.
With MyChart, you can access your billing statements and pay securely. You can also update your profile information (for example, change your address, phone number or email address).
With your MyChart account, you can review and update your communication preferences (text, email, or phone calls) for how and when you receive communications about your care.
First, you will need an email address. There are two ways to sign up for MyChart:
- Sign up online. You can create your own MyChart sign in by signing up here.
- Sign up in person. Our friendly front desk team members can help you create your MyChart account.
If you are a legal guardian or a parent of a minor under 13 years old, you can request proxy access. You must have your own MyChart account. To gain proxy access, complete and drop off completed forms with one of our Front Desk team members during your next visit for processing.
Per Washington state law:
- A minor who is 13 years old or older may initiate an evaluation and treatment for outpatient and/or inpatient mental health services, substance use disorder (SUD) treatment, or withdrawal management without parental consent.
- Minors of any age may obtain or refuse birth control and access abortion services without parental consent.
- A minor who is 14 years old or older may get testing and treatment for sexually transmitted diseases without parental consent.
- In all cases, the minor has the right to receive services in the least restrictive setting.
Optum Care Washington and The Polyclinic d/b/a Optum Care Washington (the “Practices”) are both physician owned and led practices having complete authority for all medical decision-making and patient care through their physicians and other licensed professionals.
